Social Worker (LICSW) Leominster, MA
We are seeking a Licensed Independent Clinical Social Worker (LICSW) to join our Mental Health practice full-time or part-time in Leominster, MA. We are offering flexibility of schedule (FT or PT) and the possibility for remote working (or a hybrid/combination of in-clinic and remote).
The LICSW is to utilize clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems. Duties also include, but are not limited to, conducting psychosocial assessments, making a psychosocial diagnosis, planning, and implementing effective treatments, evaluation, education, coaching and mentoring of the patient (client) and family members. This position consults with, collaborates and communicates with internal team members that include psychiatrists, psychiatric nurse practitioners, and other therapists.

Duties:
Patient consultations, assessment and intervene with high-risk patients and families
Conduct psychosocial assessments
Psychosocial Diagnosis, treatment plan and implementation
Education, coaching, mentoring of the patient (client) and family members
Consultations, collaboration, and communication with internal team members
Patient Monitoring/follow up
Requirements:
Masters Degree from an accredited school of Social Work
MA license as a LICSW
Knowledge of Social Work process, community resources and agencies relative to health, protective services, crisis intervention, disability, etc.
Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families
Previous Outpatient experience in healthcare setting preferred
Excellence in time management and staying on time with the day-to-day schedule
Computer literacy (EMR systems, Excel, Outlook, Word)
Schedule:
Full time or Part time

Full time $36-$38/hour
